Late Add Request

Students should use their CSULA email and include the completed application form and all of the appropriate parties on one email chain, ask for everyone to reply all, and then submit the final email chain and completed form to the Records Office at [email protected]. It is the student's responsibility to email Records.

Petition to Withdraw

Students should use their CSULA email and include the completed application form and all of the appropriate parties on one email chain, ask for everyone to reply all and then submit the final email chain and completed form to the Records Office at [email protected]. It is the student's responsibility to email Records.

Withdrawal Petition Addendum

Undergraduate Travel Support Form

Support for Student Travel (SUST) is a reimbursement program designed to encourage and assist NSS undergraduate students (application form). NSS encourages undergraduate students to gain knowledge and experience by presenting professional academic conference papers, posters and presentations, all of which may include travel in California and out-of-state. Students funded through SUST can receive partial reimbursement of costs for travel, lodging, and conference registration in support of their professional academic conference papers, posters and presentations . Priority will be given to students who have a paper, panel, roundtable, poster, or presentation accepted at a conference. Personal pursuits are not eligible for funding. Travel prohibited by the government, law, and/or the University are not eligible for funding. All state regulations regarding receipts submission, travel, etc., must be followed.

Overlap and Overload Forms

Students should use these application forms when they need to take two courses that overlap with one another or need to exceed 18 units (undergraduate) or 16 units (graduate) in a single semester. Please follow the instructions on each form and secure the necessary approvals, and then send the completed form to the Records Office at [email protected]. It is the student's responsibility to email the form to Records.

Graduation Application - Undergraduate Students  

In order to officially graduate, students must apply to the University for graduation. Information about how to apply, and current deadlines, can be found at this link: /graduation

Application & Diploma fee $30.00; Late fee $25.00.

Instructions for submission:

Graduation Term Update Form for Undergraduates

Use this application form to update the anticipated graduation term you originally submitted on your Graduation Application. Do not submit another graduation application. Do not pay additional graduation application ($20) or diploma fees ($10). You will not be eligible to enroll in terms beyond your anticipated graduation term until this form is processed. This form must be accompanied by a $25 late submission fee.

Instructions for submission:

  • Submit filled and signed document to [email protected]
  • Send payment by either: (1) check to the Cashier's office or (2) online

Graduation Application - Graduate Students

Credit by Examination: POLS 2000 Summary

To register, email Veronica at [email protected]. POLS 2000 allows students to test out of the California State and Local Government requirement. The online test consists of 50 multiple-choice questions that must be completed within an hour and a half. Students who do not pass the test must take one of the following courses: POLS 1000, PAS 1100, POLS 3500, or CLS 1300, or the equivalent at a California community college. The topics examined are covered in Larry N. Gerston and Terry Christensen, California Politics and Government: A Practical Approach, 14th Edition (Cengage, 2018), or a later edition. 

The book is on reserve in the University Library under POLS 2000, and may also be purchased at the University Bookstore or online in both hard copy and electronic formats at: www.cengagebrain.com. For further preparation, students may wish to contact current instructors of POLS 1000 by e-mail to inquire about auditing the section of the course that deals with California politics. Do not enroll in POLS 1000, POLS 3500, CLS 1300, or PAS 1100.
