This form may be used by the Office of Students with Disabilities (OSD) for registered students to request scanned selections of books so they may be accessed in PDF format. OSD may share this form with registered students for future use. The turnaround time will vary and is dependent on the number of pages requested and staffing availability. We will try to provide the reformatted file within 48-82 hours (about 3 and a half days). The scanned file will be emailed to the student's email address unless otherwise indicated.
