Graduation forms (two kinds - see cases):

  • Case 1: If this is your first time applying for graduation, then do this:

    Go to this page.
    Click on "Graduation Applications and Worksheets."
    Download the form "Graduation Application for Graduate/Doctorate Degrees."
    Fill out form.   It is an "MS" degree with major "Mathematics" and there is no option.
    After you fill out the form, then email it to the graduate advisor (Shaheen).
    The graduate advisor will email you back saying it's approved.
    Then you send (1) the form, and (2) the approval from the graduate advisor, to the graduation office using the email that is on the form.  

  • Case 2: If you applied previously for graduation, and need to change the term you will graduate, then you do this:

    Go to this page.
    Click on "Change graduation term"
    Download the form "Request to Change Graduation Term (Graduate)"
    Fill out the form and send it to the email that is listed on the form.
    You do not need the graduate advisor's approval for this form.

GS-10 form

Case 1 - for students doing comps:

  • If you will be doing comps, then fill out this form.   
    Fill out part 1 and sign at the bottom of part 2.   
    Then email the form to the graduate advisor (Shaheen).

Case 2 - for students doing thesis:

  • If you will be doing a thesis, then fill out this form.
    Fill out part 1 and sign at the bottom of part 2.   
    Fill out part 3 where you need a thesis name, your committee chair's info, and the names of your other 2 committee members.   
    Then email the form to the graduate advisor (Shaheen).

Withdrawal from a class:

  • Go to this page.
    Click on "Add, drop, withdraw, repeats, and other enrollment"
    Download the form "Withdrawal petition"
    Fill out the form.
    You need approvals from your instructors and the chair of the department.
    You can email the form to the instructors and the chair and they can approve it that way.
    After they give you their approval, then you send the form and the approvals to the email that is listed on the form.
