Body paint, henna tattoos, or colored powders (e.g., cornstarch)
Events or activities involving minors
Participation by non‑Cal State LA individuals in higher‑risk activities

The liability waiver is used to:
Inform participants of potential risks associated with an event or activity
Document acknowledgment and voluntary assumption of those risks
Support CSU and Cal State LA risk management and loss‑prevention efforts

A liability waiver must be used when an event or activity:
Is sponsored, hosted, or sanctioned by Cal State LA
Involves physical activity, field work, laboratory activities, demonstrations, or special events
Includes minors or non‑Cal State LA participants
Presents an elevated level of risk identified through the event or activity approval or risk assessment process
If there is uncertainty regarding waiver requirements, organizers should consult Risk Management and Environmental Health & Safety (RMEHS) prior to the event or activity.
Event and activity organizers are responsible for:
Distributing the waiver link to participants before the event or activity occurs
Ensuring all required participants complete the waiver prior to participation
Using only the approved waiver language (no edits or substitutions)
Ensuring completed waivers are retained or accessible as directed by Risk Management and Environmental Health & Safety (RMEHS)
Complying with any additional requirements identified by RMEHS
The electronic waiver link may be distributed via email, registration platforms, calendar invitations, or QR codes.
When a liability waiver is required for an event or activity, the electronic waiver process follows these steps:
Organizer Submission
Event or activity organizers access the approved electronic waiver using the link on this page and complete all required event or activity information. The form is then submitted for review.Review and Approval
Submitted information is reviewed to ensure all required details are provided. Once approved, the waiver is authorized for use.Custom Link and QR Code Issued
Upon approval, the organizer receives a custom waiver link and QR code specific to the event or activity.Distribution to Participants
Organizers distribute the approved waiver link or QR code to participants prior to the event or activity.Participant Completion
Participants complete and submit the waiver electronically prior to participation.Confirmation and Record Access
Once completed, a final copy of the fully executed waiver is provided in PDF format. Copies are available to both the organizer and the participant.
