EOP | Verification

Verification

Why does CSULA EOP require financial documents?

One of the initial criteria to be considered for EOP at CSULA is to meet the low income guidelines.
A financial verification form and document(s) showing income sources and/or eligibility for government assistance (such as a statement of state or county benefits, SSI, social security benefits, food stamps, etc.) are required in order to enable us to determine whether or not a student meets these low income guidelines. Students who do not submit these required documents by the March 02, 2014 deadline will not be considered for EOP.

What will CSULA EOP accept for a financial document?

Please see below for information regarding acceptable documents.

My Household Files Taxes. How do I know which ones to submit?

If your household files taxes, you must submit your household’s 2012 or 2013 (whichever was filed most recently) tax transcript (order from the IRS website by clicking here) or a complete 1040, 1040A, or 1040EZ federal tax return. The first page of the 1040 federal tax return will look something like this.

Important note: You must submit ALL pages including the Schedule C, SE or Schedule E (if applicable). If anything is listed on lines 12 or 17 on the 1040, you will need to submit these documents.

If your household does not and is not required to file taxes, please submit a CSULA EOP Financial Verification form as well as one of the following documents (must be current):
