Immunization Policy

Required Immunization: Hepatitis B

  • Ages 18 and younger as per California law (HSC sections 120390-120390.7)
  • Enrollees who are 18 years of age or younger are required to provide proof of full immunization against the hepatitis B virus prior to enrollment

Exemption Requests

  • Medical and religious exemptions may be requested.

The immunization requirement must be completed by the end of the first term of enrollment.

Please note: It takes approximately 7-10 business days to process documentation once entered into the Patient Portal. Students are encouraged to complete these requirements as soon as possible.

HPV Vaccination

Current immunization recommendations* advise students 26 years of age or younger to be fully vaccinated against human papillomavirus (HPV) before first-time enrollment at a California State University, University of California or California Community College campus.

HPV vaccination can prevent over 90 percent of cancers caused by HPV. HPV vaccines are very safe, and scientific research shows that the benefits of HPV vaccination far outweigh the potential risks.

Other Recommended Vaccines

  • Outbreak risk reduction: Influenza; measles, mumps and rubella (MMR); meningococcal conjugate (serogroups A, C, Y, W-135), serogroup B meningococcal; Tetanus-diphtheria-pertussis (Tdap), and varicella (chickenpox) vaccines.
  • Adults: Hepatitis A (Hep A); hepatitis B (Hep B - ages 19 and older); pneumococcal, polio, and other vaccines as applicable.
  • The following recommended vaccines are available to eligible students through the Student Health Center:
    • At reasonable cost
      • Hepatitis A
      • Hepatitis B
      • Human papillomavirus
      • Meningococcal disease
      • Measles, mumps and rubella
      • Tetanus, diphtheria and pertussis
      • Varicella
    • At no cost
      • Influenza

Additional vaccines may be required: 

  • For students enrolled in health care fields of study or teacher education programs will be required to provide immunization completion documentation as required by state licensing agencies or clinical placement or fieldwork sites.
  • In the event of a local or campus outbreak of a vaccine-preventable disease, the Los Angeles County Department of Public Health and/or the California Department of Public Health are authorized to require immunizations beyond those required by the CSU.

Submission Requirements and Instructions

All required documentation must be submitted through the Patient Portal.

All students are required to submit acceptable proof of full immunization. Acceptable proof is defined as a written document that includes immunization names and dates (e.g., immunization record) or dates and results of blood testing indicating immunity (e.g., lab report) that is signed and dated by a licensed healthcare provider.

Please note: The Student Health Center is only able to accept documents in English. Records in other languages must be translated into English by a licensed healthcare provider prior to submission to the Patient Portal.

Incoming students submit the required immunization records through the Patient Portal.

  • To access the Patient Portal, log into MyCalStateLA and click the “Student Health Center Patient Portal” icon.
    • For security reasons, be sure to log out when you are finished using the Patient Portal.

  • Click on the "Clearances and Forms" tab.

  • Proof of Full Immunization

    • Enter immunization and dates each immunization dose was received.

    • Upload immunization record documents. Be sure documents meet the definition of acceptable proof.

    • Additional records may be uploaded at any time, regardless of compliance status.

Students can check the Clearances and Forms tab for compliance status updates. Students will receive a message through the Patient Portal if any items are missing or require further action.

Please note: Submitted documents must include student name and date of birth.

The Patient Portal provides students with access to their confidential health information, including visit notes, test results, medication history, provider messages, and appointment history.

To protect the privacy of your confidential health information, please do not share your MyCalStateLA/network/email account password with anyone. If you are concerned that someone may have access to your password, please change your password at Home Page - MyCalStateLA ID.

Keep Your MyCalStateLA/Network/Email Login Password Private

The immunization requirement must be completed whether or not courses are online or in-person.

Immunization record submission must be completed via the Patient Portal.

As long as required documents have been uploaded to the Patient Portal, immunizations have been entered in the Clearances and Forms tab, your records have been received.

Students may obtain required immunizations or lab testing through their personal healthcare providers or the Student Health Center.

Students are responsible for all associated fees.

Obtaining Services Through the Student Health Center

Students who would like to fulfill the immunization requirement (or department-required lab or TB screening) through the Student Health Center should call 323-343-3300 to schedule an appointment.

Students new to the University may complete Cal State LA required immunizations, tuberculosis screening, or related lab tests prior to their first semester of enrollment under the following conditions:

  • Services are available only during the term immediately preceding the first semester of enrollment.
  • Proof of acceptance into the University and program enrollment verification must be shown prior to receiving services.
  • Students must pay applicable immunization and lab fees.
  • Eligibility for all other Student Health Center services begins with the first semester of enrollment. Please click on Eligibility for important information about Student Health Center eligibility criteria.

Student Health Center Fees

The following table identifies Student Health Center immunization and TB screening fees. Please note: Fees are subject to change.

Hepatitis B (Hep B)

$45 per dose | $6 for blood test

Three (3) doses for students age 18 and younger.

Measles, Mumps and Rubella (MMR)

$80 per dose | $22 for blood tests

Two (2) doses with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease).

Meningococcal conjugate (Serogroups A, C, Y, W-135)

$125 per dose

One (1) dose on or after age 16 for all students age 21 or younger.

TB Test: Purified Protein Derivative (PPD)

No charge | $49 for blood test

Students who are at higher risk for TB infection, as indicated by answering "yes" to any of the screening questions.

Tetanus, Diphtheria and Pertussis (Tdap)

$40 (no charge if injury related)

One (1) dose after age 7.

Varicella (Chickenpox)

$150 per dose

Two (2) doses with first dose on or after 1st birthday; OR positive titer. History of contracting the disease does not meet compliance.

The Student Health Center is only able to accept documents in English. Records in other languages must be translated into English by a licensed healthcare provider prior to submission to the Patient Portal.
