COVID-19 Student Self-Certification Instructions

Instructions for COVID-19 Student Self-Certification
Step 1: Log in to the MyCalStateLA portal.
Step 2: Under the Applications section, click on Student Health Center Patient Portal and sign in with your MyCalStateLA ID username and password.
Step 3: On the Student Health Center Patient Portal homepage, follow the instructions under Covid-19 Vaccination, Enrollment Immunization and Screening Requirements for Incoming Students and click on Clearances and Forms on the left-hand navigation bar.
Step 4: Update your COVID-19 vaccination record or request for a medical or religious exemption.
For updating your vaccination record:
- 1) Click on the Update button for COVID Vaccine under Items required for clearance.
- 2) Under COVID-19 Vaccination Card Upload, upload a copy of your proof of vaccination document per the instructions on screen.
- 3) Under COVID-19 Vaccine History, add your vaccination details per the instructions on screen.
- 4) Click Done. You will see a confirmation message that your immunization changes have been successfully saved.
NOTE: All compliance status updates will be reflected on the Clearances and Forms tab once records have been processed by the staff.
For an exemption request:
- 1) Click on Downloadable Forms on the left-hand navigation bar or click on the Downloadable Forms link on the Clearances and Forms page.
- 2) Under either COVID-19 Exemption Request – Medical or COVID-19 Exemption Request – Religious, click Preview or Download to review the instructions.
- 3) Click the Upload button to upload your supporting documentation.
- 4) Once your file has been attached, scroll down to select Save. You will see a confirmation message once your file has been successfully uploaded.
NOTE: All compliance status updates will be reflected on the Clearances and Forms tab once records have been processed by the staff.
HELPFUL REMINDERS
- Your updated proof of vaccination that includes your booster should include:
- The date(s) you received each of your vaccination doses, including the booster.
- The name of the vaccine you received.
- Your full name and birth date, which should match your Cal State LA information in GET.
- Supported image types include: .jpg, .jpeg, .pdf, .png, .jfif, .tiff, .bmp
- Alternatively, you can attach your official state digital vaccination record as your proof, which you can access on the state of California's Digital COVID-19 Vaccine Record portal. Be sure to include the QR code and all of your vaccine administration information, rather than just the QR code.
