COVID-19 Student Self-Certification Instructions

See below for instructions about how to verify your vaccination and booster status through COVID-19 Student Self-Certification. Find frequently asked questions about the vaccination requirement.

Instructions for COVID-19 Student Self-Certification

  • Step 1: Log into the MyCalStateLA portal.
  • Step 2: Under the Academics section in the portal, click on GET.

  • Step 3: Once in the GET system, select Main Menu, select Self Service, and then select COVID-19 Self-Certification. You can follow this path in the top dropdown menu or on the Self Service page.

  • Step 4: Read the information and instructions on the page and select the appropriate options.

  • For more information about terminology, click the hyperlinked words.
  • Step 5: If you are uploading your vaccination or booster shot proof, make sure the Self-Certification of COVID-19 Vaccination Status section is checked.

  • Step 6: In the box that appears, select the manufacturer/type of vaccine you received. Then enter the information for your first dose, second dose (if you received a two-dose vaccine), and booster.

  • Step 7: Make sure the Self-Attestation section is checked.

  • Step 8: Click the Attach button to attach your proof of vaccination or supporting documentation for an exemption request. A request for a medical exemption must be accompanied by documentation from a certified or licensed healthcare professional. A request for a religious exemption must be accompanied by a personal statement or a statement from a religious leader that describes the applicable religious or other comparable belief that is the basis for the exemption request. All exemption supporting documentation should include your name, CIN (campus identification number), and the type of exemption you are requesting. If you are updating your booster information: In the Attachment section, delete your previous attachment and attach your updated proof of vaccination that includes your booster. Important: Do not delete your original attachment until you have your updated proof ready to upload.

  • A pop-up will prompt you to select your file and then upload.

  • Now you will see the name of the attached file and the options to view or delete.

  • Step 9: Click Submit at the bottom of the page and you will be taken to a confirmation page with a thank you message.

Helpful Reminders

  • Your updated proof of vaccination that includes your booster should include:
    • The date(s) you received each of your vaccination doses, including the booster.
    • The name of the vaccine you received.
    • Your full name and birth date, which should match your Cal State LA information in GET.
  • Supported image types include: .jpg, .jpeg, .pdf, .png, .jfif, .tiff, .bmp
  • If your booster shot proof and your proof of receiving your first doses are on separate vaccination cards, you must combine them into a single digital document for the upload. Alternatively, you can attach your official state digital vaccination record as your proof, which you can access on the state of California's Digital COVID-19 Vaccine Record portal. Be sure to include the QR code and all of your vaccine administration information, rather than just the QR code.
