  • Do not include information irrelevant to the decision. For example, information such as directions to a parking lot should be on a separate sheet.
  • Do not use I-statements but write the entire document in the second person.
  • Remember to disclose if the manufacturer is also the one funding the study.
  • Do not use the word "informed" in the title of the document. Just use consent with the title of your study.
