Testing Accommodations

Midterms, Exams and Quizzes

Must submit completed proctor form one-week prior to test date.  Morning classes must be scheduled for 9:00am, Afternoon classes must be scheduled for 1:00pm, and Evening & Nighttime classes for 5:00pm.  If have exams/quizzes on Saturday you may request to take the exam/quiz on Saturday.

Final Exams

You must submit a completed proctor form by November 23rd to receive testing accommodations for Final Exams. Morning classes must be scheduled for 9:00am, Afternoon classes must be scheduled for 1:00pm, and Evening & Nighttime classes for 5:00pm.  If  your final is scheduled for a Saturday you may request to take the final on Saturday.

Proctor forms will not be accepted after the deadline.

NO EXCEPTIONS!

Tests must be arranged with your instructor

• To reschedule a test, the instructor must approve the change, they must notify the Office for Students with Disabilities (OSD), and OSD has the right to refuse the accommodation if we are unable to provide the service

• It is the student’s responsibility to ensure that all forms are completed and submitted on time to OSD
