2013 Student Evaluation for the OSD
This comprehensive survey covers all areas of services provided under the auspices of the Office for Students with Disabilities. From disability accommodations, notetaking services, testing accommodations and specific programs and activities. Estimated time for taking the survey is 7 minutes.
The Alternative Media Production Center provides students with Braille and electronic books and curriculum. The AMPC Survey covers ares of quality and timeliness of services. Takes 2 minutes to enter responses.
The Assistive Technology and Learning Center provides a supportive lab for student. In the ATLC Survey students can evaluate the work done by staff and make suggestions for program improvement. Takes 3 minutes to complete this survey.
The TRIO program supports disabled students with additional resource provide by a federal Dept. of Education grant. The TRIO Survey is a way for students in this program to provide feedback to the OSD about the quality of services. Estimated time to complete this instrument is 5 minutes.
Captioning and Interpreting Survey for Instructors
The Captioning and Interpreting Survey allows instructors at CSULA to provide feedback about their service providers who worked in their classrooms. The nine questions also include information identifying which class is being reviewed. It will take about 3 minutes to complete the survey.
Service Provider Survey for Students
The Service Provider Survey allows students to provide feedback about their service providers. It will take about 3 minutes to complete the survey.
OSD Testing Survey
This 5-minute OSD Testing Survey asks for student feedback for their experience with using OSD testing accommodations.
