- General Release Forms - College or department initiating use of either general release form should retain original for one year and send copy/copies to the Office of Risk Management.

- U.S. State Department Updates: The attached file lists countries designated in the following risk areas: High Hazardous Countries; Excluded Countries (no travel allowed); and Countries the U.S. State Department has issued a 'Travel Warning' for. All of the aforementioned countries require approval from the CSU Chancellor prior to travel. The traveler should always consult the Risk Management Office [http://www.calstatela.edu/univ/ehs/rm_travel.php] or the U.S. State Department website [http://travel.state.gov] for updated/most current information.
