Faculty Web Page Request Form

This form may be used by faculty who wish to have a web page automatically generated for them on the University web server based on a pre-existing template (view sample). Once you submit the form, the Web Administrator will create a web directory, place the newly generated web page into that directory, and add the page to the Faculty Web Directory listing. You will be notified by email when this process has been completed. Requests may take up to 3 business days to process. If you have any questions, you may contact the Web Administrator at wwwadmin@calstatela.edu.

Note: All users who need to access the University web server must have an NIS account.
