Software Installation Request Page

Installation on baseline computers only.

This form should be used to request the installation of Acrobat X Pro on only faculty and staff assigned baseline computers. Requests with missing information may delay the installation of the software.

Procedures

1. Fill out and submit the form below.
2. Upon submission, you will receive a confirmation e-mail.
3. You may be contacted for further information or instructions.
4. Use the following to find out your Computer Name:
• PC - Click Start. The computer name is the same as "My Computer" on that page. (EXAMPLE)
• Mac - Click Apple to System Preferences then select Sharing. The computer name appears as "Computer Name" on that page.
