Software Installation Request Page

Installation on Windows XP SP3 or greater computers only.

This form should be used to request the installation of SAS on any Windows XP SP3 or greater campus computers.

Notes:

• Please have computers connected to the campus wired network and turned on.
• Requests with missing information may delay the installation of software.
• The software will be installed on your baseline provided computer no later than five business days.

Procedures

1. Fill out and submit the form below.
2. Upon submission, you will receive a confirmation e-mail.
3. You may be contacted for further information or instructions.
4. Use the following to find out your Computer Name:
• PC - Click Start. The computer name is the same as "My Computer" on that page. (EXAMPLE)
