This form should be used to request the installation Office 2011 for Mac on any Mac OS X 10.5.5 or greater campus computers

Requests with missing information may delay the installation of software.
The software will be installed on your baseline provided computer no later than five business days.

Use the following to find out your Computer Name:
- Mac - Click Apple to System Preferences then select Sharing. The computer name appears as "Computer Name" on that page.
- Mac - Click Apple to System Preferences then select Network. The IP address appears in the middle box.
