Forms for Teacher Consultants:  presenters should use these forms when presenting inservices at schools or districts. Send to Carolyn at the Los Angeles Writing Project (* Note: ALL documentation is needed in order to pay Teacher Consultants for inservice presentations. Adobe Acrobat Reader is needed for ".pdf" documents)

	UAS Vendor Form  (Contact info.,Signature and #5 ONLY)
	Teacher Consultant Invoice  (please attach email documentation of amount owed)
