Procedures for the Administration of
Student Opinion Surveys

1. Prior to the ninth week of the quarter, the University shall make up a survey packet for each class to be surveyed. Included in the packet will be the necessary number of blank survey forms and marking pencils, an instruction sheet for the students who administer the surveys, and an envelope addressed to the Survey Coordinator, Administration 410, into which the completed forms are to be placed. The packets are sealed and an instruction sheet for the faculty member is attached to the front of the packet. The packets are then placed in the faculty member's boxes as required.

Those departments/divisions/schools offering classes at off-campus locations must include with the survey packet a postage-paid, pre-addressed envelope for return of the completed forms by mail to the Survey Coordinator.

2. During one class period in the ninth or tenth week of the quarter, the faculty member shall take the packet to the class and shall provide a 15 to 20 minute block for administering the survey, under the direction of one or more students, as instructed on the packet attachment. The class instructor may request his/her department/division chair or school coordinator to appoint another person to proctor the administration of the survey. Without any further comment, the class instructor shall leave the room until the survey process is completed.

3. After the class completes the survey, the designated student(s) shall place the forms in the addressed envelope, seal it, and deposit the sealed envelope in specially designated campus mail depositories as shown on the map accompanying the instructions.

If the class is being held at an off-campus location the students administering the survey must place the forms in the postage-paid, pre- addressed envelope, seal it, and deposit it in the U.S. mail.

The Survey Coordinator shall keep the forms in a secure place until they are processed. A copy of the summary for each class surveyed shall be sent to the faculty member, the department/division/school, and the dean of the college after the grading deadline. Original data sheets shall be returned to the instructor sealed, and marked confidential.

The attached envelope contains a Student Opinion Survey for each student in the class, as well as an instruction sheet.  University policy requires that these materials be distributed to the appropriate faculty members during the eighth week of the quarter to allow the survey to be conducted during the ninth and not later than the tenth week of instruction.
In class, a 15- to 20-minute block is to be set aside for the purpose of administering the survey in accordance with the instructions inside the envelope. Select one or more students and inform them that they are to administer the survey.  Indicate to the class that a designated student will read aloud section two of the instructions in the survey packet.

INSTRUCTIONS FOR ADMINISTERING STUDENT OPINION SURVEY

 
One or more students shall administer the Student Opinion Survey within a 15 or 20 minute block set aside during the ninth or tenth week of instruction. The instructor must be absent from the classroom during the entire survey process.
1. Start by counting and recording below the number of students present in class and the date.
2. Read the following statement to the class, and make no further comment:


"Student surveys are required by the University as one of the means, but not the only means, of judging the effectiveness of instructional performance. The professor does not receive these survey forms until after turning in final grades. Silence is to be maintained during the administration of this survey.

"Space is provided on the form for comments you may wish to make directly to the instructor regarding course content, method of presentation, materials, assignments, or any other area of instructional performance."

3. Distribute forms and pencils.
4. By the end of the survey time period: a) sign this sheet; b) collect the forms; c)  place all forms, used or unused, pencils, and this instruction sheet in the envelope. Detach the notice of student survey from the outside of the envelope and place it inside as well.
5. Seal the envelope containing the surveys and related materials.
6. Immediately after the class is concluded, put the sealed envelope in the specially designated campus mail depositories as shown on the map.

If the class is being held at an off-campus location, place the surveys and this form in the postage-paid, pre-addressed envelope provided and deposit in the U.S. mail.
