Credential Evaluation Forms How to Apply for a Credential? Are you ready to apply for your credential? The answer is yes only if you have completed all your credential requirements or are enrolled in the only remaining credential requirements this quarter. If you are ready to apply for your credential, follow the steps below. Please carefully read steps 2 – 8 below and then select the appropriate credential you are applying from the list below. Type all the information on the form (handwritten forms will not be accepted). Attach photocopies, of all required documents, to the typed credential processing and evaluation forms. Also, verification of experience and examinations requires that you bring in the ORIGINAL documents, as well as photocopies. Submit completed form and attached requirements to Student Services KH D2078. FAX applications will not be accepted. If you have any outstanding requirements once an evaluation is completed, you will be sent a letter indicating the outstanding requirement(s). Once all requirements have been verified, your application with the University’s recommendation will be electronically sent to the California Commission on Teacher Credential. Please check your e-mail for further instructions from the Commission so that you can complete your portion of the credential application processing. PLEASE NOTE: The Credential’s Office no longer issues C-19 letters for electronically recommended credentials. If 6 weeks have elapsed from the end of the quarter you completed ALL of your credential requirements and you have not received an e-mail with instructions from CTC to complete your portion of the credential processing, please follow these instructions: Step 1. Go online and type in the following web address for the student login: https://teachercred.ctc.ca.gov/cctc_phase3/InteractionMgr?interactionmgr.interact ion=Enter_ApplicantLogin Step 2. A page entitled Credential Recommendation – Applicant will be visible with a box entitled Applicant Log In where you will need to enter your social security number and date of birth. Follow the instructions and enter all additional information (response to Character and Identification Clearance questions, address, credit card number, etc.) Step 3. When all the necessary information has been completed; a Confirmation Page will be displayed. You should print out the Confirmation Page as a receipt. Credential application deadlines for 2008 - 2009 academic year are: FALL QUARTER, 2008 Thursday October 23, 2008 WINTER QUARTER, 2009 Thursday February 5, 2009 SPRING QUARTER, 2009 Thursday April 30, 2009 SUMMER QUARTER, 2009 Thursday July 23, 2009 Credential applications are processed in the order they are received. Education Specialist Credential Evaluation & Processing Form Use this form to apply for a Education Specialist credential. Education Specialist Credential Evaluation & Processing Form Use this form to apply to have the EL (English Learners) Authorization Added to a valid Education Specialist Level I or Level II. Verification of Employment as Education Specialist(CL-777.1) Use this form to verify employment as Education Specialist for Level I. Verification of Experience as an Education Specialist (41-EXP) (Level II) Use this form to verify teaching experience as an Education Specialist for Level II. Administrative Services Credential Evaluation & Processing Form Use this form to apply for an Administrative Services Credential. Verification of Employment as an Administrator (CL-777) Preliminary Use this form to verify administrative position for the Preliminary Credential. Verification of Experience as an Administrator (CL-41) Professional Clear Use this form to verify experience as an administrator for the Professional Clear Credential.
