If you are taking classes toward earning a certificate, credential, or degree in one or more programs in the CCOE, please complete the survey below for the program(s) in which you are taking classes. Each survey should take less than 20 minutes. Once you complete a survey and push the SUBMIT button, the “Thank You” page will be visible. Please print copies of the “Submission” page for each CCOE class that you are enrolled in, print your name on each copy, and give one to each of your course instructors for recognition of participation.
