To obtain Institutional Animal Care and Use Committee (IACUC) review and approval of research and teaching projects using live, vertebrate, non-human animals, please fill out and submit a protocol application form as per the following instructions. Completion of these forms is required by Federal agencies and University policy.

Deadlines: Request for instructional and research use of animals should be submitted to ORAD at least 30 days prior to the IACUC meeting (see the IACUC Meeting Schedule).
